Some improvements to game startup:

-fade out screen before applying window/res settings (which looked ugly
 when "Loading..." was shown off center, and garbage on-screen
-fix the mouse cursor state not being reset when leaving a game
-hide upgrade messages if using rungame.
-I removed the box behind "Loading..." because it looks a bit odd
 with nothing else on the screen
