Add a global sound effects volume level to allmodex.

The playsfx volume setting is now a multiplier on the default volume
for a sound effect.

Added several new functions to allmodex and music backends, and
allow allmodex to store per-slot data directly in the backend
slot arrays by using a common TYPE.
