Yikes, the UTF8 code in unicode.c was quite broken due to signedness errors

Turns out that I did a fair amount of adaptation of the original code when
importing it, and didn't test it.

This explains why decode_filename() wouldn't work when I forced it to interpret
filenames as UTF8.
