In Custom allow Insert or Numpad + keys as alternative to +, or vice versa, nearly everywhere

These were already allowed as alternatives in many places. Under gfx_sdl2
the + key can be hard to find on a non-US keyboard layout (but many other keys
have the same problem)

Also Numpad- instead of - in one place.

And when you insert a new hero battle menu item, put it at the current cursor
position.
