spawn_and_wait: combine Mac and Unix codepaths, using a wrapper shell script to run the program

Refactoring to enable the next commit (capturing the exit code), but not having
a wildly different codepath on Mac is a nice benefit.

Deleted the stdout & stderr capturing of the Unix path, which wasn't useful
except hypothetically xterm printed something because it couldn't start.
