Fix ModularMenu mouse hover jumping when the menu refreshes

Also, init_menu_state no longer clobbers .hover or .rect.x/y.

The ModularMenu bug was due to not passing menuopts to init_menu_state, so
itemspacing was wrong.  This also fixes the General Settings Menu problem where
the scroll position jumped back up when you tried to scroll down to see Stats.
