Ensure use background color isn't used for text hover or text UI colors

This is significant for very small palettes, where black often got selected as
some the default UI colors, and also sometimes the selected mouse hover color.

nearcolor()'s avoidcol argument now ignores all master pal colors equal to that
RGB triple rather than that index.
