Looking again, it turns out that the first two one-time tags *are* explicitly excluded starting in r5458 (34cec45) because onetime bits 0 and 1 don't work. San must have set the NPC to an invalid one-time tag using an older build (it was already fixed at the time and in Beelzebufo).
