Add Ctrl-B all-bits debug editors to item and enemy editors

Added show_index and show_all/show_hidden args to editbitset, and a
edit_all_bits wrapper around editbitset (in lieu of creating a third overload).

I didn't add a Ctrl-B key to the hero editor because no extra bits are saved
in heroes.reld, so such a debug menu would be almost useless.

Also renamed "Healing poison causes regen and reverse" to
"Healing poison causes regen, and vice versa"
