Add explicit ACCESS_READ/ACCESS_WRITE flags to OPENFILE calls that didn't have them

(except INPUT/OUTPUT/APPEND opens, which don't use ACCESS)

There were a couple more places where an output file wasn't truncated because
ACCESS_WRITE was missing, e.g. in bam2mid, but they seem pretty harmless.
