Ensure .initiative_order is only added once to non-blocking queued attacks in turn-based mode

Such attacks can potentially be delayed until a later round/turn.

Still one known problem: if all blocking attacks in a round have a negative
delay, then any non-blocking attacks will have their .delay increased,
potentially going on forever
