Strip down fb/error.c, producing format_FB_error_message() from fb_Die()

I didn't want to manually duplicate the mapping from FB error numbers to error
descriptions (or the formatting code) that exists in FB's src/rtlib/error.c, but
there's no way to access it. I think copying in code from FB is easiler to
maintain.
