Move all functions I could find for generic caption-with-default to util.bas, and homogenise their arguments a bit

The main redundancy is between zero_default and defaultint (which defaults to
-1 as indicating default instead of 0), but that seems sortof ok.
I wouldn't be surprised if there are more similar functions I didn't find.
