menus: Add correct_menu_state (replacing clamp_menu_state), and use it everywhere

This just clamps .top and .pt to valid values, and is called after changing
the menu size or .pt.

Not used in a couple init_menu_state overloads because they keep .pt
away from the very top or bottom of the screen. All usemenu overloads
should have to do that too before changing correct_menu_state, which
would be nice, but I don't want to get too sidetracked right now.
