Remove many manual assignments to state.spacing, which gets overwritten by standardmenu

Also set .autosize=YES in a few menus, removed some manual .size recalculations,
and switched a few menus to .[fullscreen_]scrollbar=YES instead of drawing it
manually.

There are just two menus which don't use standardmenu and so still set
state.spacing themselves. (There are probably many more which reimplement
standardmenu and don't touch .spacing at all)

standardmenu now also calls recalc_menu_size, since usemenu can't do it on the
first tick (leads to graphical glitches), and usemenu may not always be called.
