Per-textbox each-line sfx option (Patch by MirceaKitsune/Taoki)

Overrides gen(genTextboxLine) option if nonzero.
.SAY lump extended.

I modified the patch by fixing a bug, displaying and playing (on Enter)
the global sound effect in the appearance menu, and also documenting the
global setting.
