Set SDL_DISABLE_LOCK_KEYS, and change the bug 619 workarounds so that old SDL versions are also playable. The pause key in battle isn't quite right, but gfx_fb and gfx_sdl with SDL <= 1.2.13 both actually behave the same now for me in Linux. Haven't tested in Windows.
