I currently suspect the volume fader is the problem. Looking at the code in
music_sdl.bas I see comments that suggest that the current fading
implementation is just a messy hack, and the way it fades using blocking
"sleep" calls suggests to me that it could causes thread locking problems.
My preliminary tests indicate that if I simply disable volume fading support,
the ogg music plays correctly and the crash goes away.
